When will enough people realize that for-profit medical insurance is by definition fraud. Either the company is defrauding shareholders by paying too many claims or defrauding customers by not paying enough claims. At any given moment one of them is being defrauded.

As I’ve stated (not the first or last) employer sponsored health insurance is the worst thing to happen to the US. What started as a “fringe benefit” to employees has morphed into a chimera. Companies used to be self insured but went away from that and started contracting to insurance companies. When I started in industry about 30 years ago I was working for one of the largest pharmaceutical companies and it was a self insured plan. My third year we were converted to Cigna.

Kaiser started as the healthcare arm of Kaiser Steel, insuring those who worked on building ships - the parent doesn’t exist anymore!!
