Before passage of the ACA in 2010, every individual state set their own rules on pre-existing conditions. Ins. co.s were also allowed to charge higher premiums and put annual &/or life-time caps on coverage for each pre-existing condition. And, depending on the state, insurance co.s used exclusion periods and elimination riders so they could avoid paying for people with pre existing conditions’ health coverage. So people with chronic conditions, i.e., the people who need insurance the most, could run out of benefits and be stuck paying huge medical bills even though they had medical insurance. During the health care reform debate, a congressional investigation discovered that more than 400 medical conditions or diagnoses were used to justify denials of coverage.
These regulations mainly applied to the individual health insurance market, not the group plans that many people join through their employer. In fact, many Americans did not even realize the individual market was different until they lost their jobs and their group coverage. A loss of group coverage meant they had to pay the entire premium themselves and this made it difficult for them to find affordable health insurance and many were priced out of affording healthcare because of their pre existing conditions.

I know you're not stupid. So I know that you know how they worked this out. Instead of charging those of us with pre-existing conditions "high-risk" premiums, they allowed/compelled insurance companies to charge EVERYONE high-risk premiums. THAT'S what "guaranteed coverage" means, and that's why premiums have skyrocketed. If insurance companies are obligated to cover the guy who found out yesterday that he has ALS, then they have to make up the cost somewhere. And that cost is at EVERYONE else's expense.
That's a big part of the reason why the ACA is thoroughly dysfunctional.
But then, you knew that.

Back to the regularly scheduled demagoguery: the bigger the risk-pool the lower the incremental cost, but you know that also. We figured out rocket science but the political class bought and paid-for by the insurance lobby (and contrarian hold-outs) won't allow us to implement what is taken for granted in the rest of the civilized world.
